FIRST STEP		 nomination by home university
The SECOND STEP filling in on-line application form
 
Learning Agreement for studies https://learning-agreement.eu/  and upload PDF with all 3 signatures: Student, The Right Coordinator from Home Institution, The right Coordinator from Host Institution.
Learning Agreement  for traineeships signed by you and Erasmus+ coordinator from your University 
Please send above listed document before deadline:
  • 31 may 2021 -  for winter semester, for whole academic year
  • 30 November 2021 – for summer semester

After you upload documents from SECOND STEP successfully, you will be able to upload the remaining required documents.
The
THIRD STEP		 Upload other documents (until 15 September 2021 for 1st semester and whole year) and (until 31 january 2022 for second semester and for summer traineeship)
  • scan of id card (or passport)
  • scan of European Health Insurance Card or any relevant one
  • scan of original confirmation of accident insurance
  • scan of original liability insurance (mandatory for traineeships, optional for studies)
  • scan of Erasmus+ status certificate (issued by your home university)
  • scan of student's statement
The FOURTH STEP Registration after arrival. Due to covid-19 pandemic we have to change some rules:
Do not come to Erasmus Office personally (all needed documents you have to send by e-mail to dominika.lucinska@umed.wroc.pl).
When your student's id is ready you will be informed about the date to collect it (we're going to divide you into small groups max. 15 students per day).

Documents to be sent by e-mail are as follows:

  • General data protection Rodo 2020
  • Proof of payments for student's id https://www.umw.edu.pl/erasmus-students-id
  • Certificate of arrival if applies (attach to mail and write the date of your arrival, I will confirm that and send it back to you)
